relationship (one of the following is required):

Duly
executed adoption documents

Duly executed Court Custody documents

Notarized affidavit
from non-resident custodial parent along with notarized affidavit
from non-custodial parent*
2. Child’s Social Security Number
3. Child custody documentation (if applicable)
4. Child’s Health Certificate (physical) and immunization record
5. Name and mailing address of school last attended
6. Child’s last report card
7. Proof of parent’s residency in Nyack School District (three
forms of the following are required):

Notarized Affidavit from the
Landlord with proof of ownership*

*Affidavits may be obtained from the Registration Office and must be
completed prior to registration.
